Right at Home Ottawa is looking for a PSW Team Lead / Junior Care Planner to support our growing team of caregivers and clients across the community. This role is perfect for someone who enjoys a balance of client interaction, care coordination, and team mentorship while contributing to exceptional service quality and client satisfaction. Role Description In this full-time, hybrid role based in Ottawa, the PSW Team Lead / Junior Care Planner supports both clients and caregivers to ensure high-quality, compassionate home care. Key responsibilities include: Coordinating day-to-day care delivery by working closely with the Scheduler and Care Planner. Conducting client assessments, developing personalized care plans, and updating them as needs evolve. Supervising and mentoring PSWs, providing ongoing feedback and coaching to uphold care standards. Acting as a primary contact for clients and families, ensuring effective communication and timely support. Participating in recruitment, onboarding, and training of new caregivers. Maintaining accurate documentation and reporting in the tools used. Qualifications Certification in one of the following: Personal Support Worker (PSW), Social Service Worker, Occupational Therapist Assistant / Physiotherapist Assistant, or recent RPN graduate (eligible to practice in Ontario). Post-secondary education in a related field (e.g., healthcare, social services, business, or administration) is preferred. 2–3 years of experience in home care, community health, or administrative coordination. Strong leadership and interpersonal skills with the ability to mentor and support caregivers. Proficient in Microsoft Office Suite (Word, Excel, Outlook) and comfortable using client databases and scheduling systems. Excellent written and verbal communication, organizational, and time management abilities. Current First Aid and CPR (Level C) certification and Vulnerable Sector Check (issued within 6 months). Valid driver’s license, reliable vehicle, and proof of insurance required.
