Job Description Job Description ABOUT GLYCARE GlyCare is a hospital-based group practice specializing in inpatient diabetes and glycemic management. Under the direction of our Medical Director, GlyCare Nurse Practitioners deliver high-quality, evidence-based, protocol-driven care to hospitalized adults in non-ICU settings. As a GlyCare-employed NP or PA practicing under consulting privileges, you’ll combine independent patient management with a strong commitment to quality improvement, teamwork, and adherence GlyCare standards of care — improving outcomes across the hospitalized population. Full-Time: Mon–Wed every other weekend OR Wed–Fri opposite weekends Part-Time: Mon–Tue every other weekend OR Thu–Fri opposite weekends (~12 shifts/month, with full benefits) Predictable Workday – Start 7:30 a.m., finish by 4–4:30 p.m. (exception for surge census or short-staffed periods) Impactful Role – Dedicated to inpatient diabetes care for adult patients on Med-Surg and Step-Down units (non-ICU level of care) Autonomy Support – Independent rounding with protocol guidance and endocrinologist support. POSITION DETAILS Schedule Types: Full-Time: Mon/Tue/Wed every other weekend OR Wed/Thu/Fri opposite weekends (~52 fewer shifts/year vs. M–F jobs). Part-Time: Mon/Tue every other weekend OR Thu/Fri opposite weekends (~12 shifts/month, full benefits included) Position Locations and Openings: St. Joseph Hospital – Bethpage, NY: 2 Part-Time must be able to start 11/10 St. Catherine of Siena Hospital – Smithtown, NY: 3 PT must be able to start 11/17 St. Charles Hospital – Port Jefferson, NY: 1 FT must be able to start 12/1 Compensation: Base Pay : $60–$72/hour, commensurate with inpatient hospital practice experience, education, and qualifications. Monthly RVU Bonuses : Uncapped, based on productivity and quality 100% Employer-Paid Health Insurance (PPO plan) 401K Long/Short-Term Disability Dental/Vision PTO $600 iPhone stipend What You’ll Do Provide consultative inpatient diabetes/glycemic management (non-ICU). Manage a daily census of ~20–25 patients (higher census = 28–30). Independently round using validated protocols with endocrinology oversight. Rotate after-hours call coverage (phone only, no return to facility). Engage in ongoing professional development via GlyCare University. Qualifications Active, unrestricted New York license as a Nurse Practitioner (FNP, AGNP, or ACNP) or Physician Assistant (PA-C) . Completed in-person clinical training hours. Previous inpatient nursing (for NPs) or inpatient/acute care experience (for PAs) required. Recent experience practicing as an NP/PA in a hospital (preferred) Medicare provider number preferred (not required). Strong clinical judgment, efficiency, and ability to work independently within protocols. EMR proficiency (Epic or similar) with ability to learn quickly. Minimum 2-year contract commitment. Professional Standards & Expectations As part of GlyCare’s hospital-based group practice, APPs are expected to: Commitment to patient-centered, evidence-based care in alignment with GlyCare protocols. Strict adherence to HIPAA and confidentiality standards. Reliability with schedules, weekend/holiday coverage, and call rotations — continuity of inpatient care is essential. Participation in required training, chart reviews, and quality assurance activities through GlyCare University. Collaboration with interdisciplinary teams, maintaining professional communication and responsiveness (e.g., timely follow-up on patient care and chart reviews). Flexibility to assist colleagues or cover other facilities if needed to maintain patient safety. Additional Notes CVs must explain any gaps in education through first clinical position and/or practice experience ( required for privileging). New graduates (NPs and PAs) are encouraged to apply.
