Job Description

Job Title: Registered Nurse Profession: RN Specialty: LTAC Med Surg Duration: Permanent Shift: 12 Hour Nights Hours per Shift: 36 hours per week Experience: Minimum six months of Medical/Surgical experience in an acute care setting preferred. Will review candidates with 1 year experience. New grads will be reviewed on a case-by-case basis. License: Current state licensure as a Registered Nurse. Certifications: Basic Life Support (BCLS) certification required; Advanced Cardiovascular Life Support (ACLS) preferred. Must-Have: Excellent oral and written communication skills. Knowledge of medications and their correct administration based on patient age and clinical condition. Ability to organize tasks and develop action plans in stressful situations. Must maintain good attendance. Description: Provides planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, intervention, and evaluation. Develops a nursing care plan in coordination with patients, families, and interdisciplinary staff as necessary. Communicates changes in patients' clinical conditions appropriately. Participates in the discharge planning process. Maintains standards of nursing care and implements policies and procedures. Directs, supervises, and evaluates nursing care provided to patients. Assigns nursing care team members based on patient needs and team capabilities. Documents patient admission assessments and re-assessments, care plans, and other pertinent information in the patient's medical record. Performs assessments on all patients upon admission and re-assessments as per policy. Makes referrals to other disciplines based on assessment findings. Develops and updates nursing care plans for assigned patients as needed. Assesses and reassesses pain, utilizing appropriate pain management techniques. Educates patients and families regarding pain management strategies. Revises the plan of care according to patients' responses to treatment and evaluates overall effectiveness daily. Receives and ensures accurate transcription of physician orders and documents task completion. Assists physicians with examinations, treatments, and special procedures. Administers medications accurately utilizing the five rights of medication administration. Formulates a teaching plan based on identified learning needs and evaluates its effectiveness, involving family as appropriate. Interacts professionally with patients and families, involving them in care planning. Treats patients and families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Functions as a liaison between administration, patients, physicians, and other healthcare providers. Performs other related duties as assigned.
