Job Description Job Description UCP Heartland, is looking for an LPN for our Adult Day Program in the Crestwood, MO Area At UCP Heartland our mission is to provide children and adults living with differing abilities extraordinary care and support they need to thrive in school, at home, at work and in their community. Do you have: Your LPN in Nursing from an accredited college, university or hospital Required licensure and registration with the state of Missouri. One Year of experience working with people with developmental disabilities Expertise and familiarity with electronic records systems as well as basic computer software Excellent written and verbal communication skills Ability to transfer adults who may weigh in excess of 150 pounds using mechanical supports. We have some fantastic benefits: Generous vacation, sick and personal time off policy PLUS 10 Company Holidays Excellent benefits to include medical, vision, dental and life insurance (COMPANY PAID VISION) Short-Term and Long-Term Disability OUTSTANDING retirement plan Advancement opportunities So, what does the job look like? • The primary nurse in either the Community Services – Crestwood (with flexibility to travel and work at our North location as needed) • Ensure the physician orders are present, current and inclusive of all required information per Agency and DMH policies. • Administer medications and/or treatments as prescribed (g-tube feedings, nebulizer treatments, etc.) for all Clients ensuring prompt, thorough documentation in accordance with Agency policy and accepted standards of care. • Assures medication storage and disposal in accordance with Agency requirements. • Provide care for Clients in need of first aid or more advanced medical services or who are otherwise ill. • Daily charting of medications and Client health information using an electronic records system. • As needed, provide for and assure that personal care needs (ADLs) of Clients are met. • Facilitate and monitor Clients’ special diet orders. • Assure compliance with Agency policy regarding medical protocols (annual physicals and updated medical records) for Clients in coordination with the RN and Agency’s Medical Director. • Interact on a professional level with families and physicians to assure that Client needs are presented and addressed. • Provide staff training in the areas of health management, medication administration, and infectious disease control among others. • Acts in the absence of the Registered Nurse. We Would Prefer You Have: Three or More Years of experience working with people with developmental disabilities. This is a FULL TIME (40 Hour/Week M-F), NON-EXEMPT POSITION.
