Our interdisciplinary team of medical and health professionals, including physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists, is dedicated to keeping residents healthy, active, and living comfortably at home. Role Description This is a part-time, on-site role for a Nurse Practitioner located in Whitby, ON. The Nurse Practitioner will be responsible for providing primary care to residents, conducting health assessments, developing care plans, and coordinating with the interdisciplinary team. Additional responsibilities include managing chronic conditions, providing preventative care, and responding to acute health issues. Responsibilities Primary/Urgent Care for Seniors: Serve as the main healthcare provider for patients, offering timely assessments, diagnoses, and treatments for a wide range of medical concerns. Provide on-site services at the retirement residence in Oakville and offer virtual follow-ups as needed. Patient Consultations and Assessments: Conduct thorough consultations and assessments to determine the best course of treatment tailored to each client's individual needs. Health Status Documentation: Assess and document patient health status and needs accurately. Care Plan Development: Develop and implement individualized care plans based on assessment findings. Medication and Treatment Administration: Administer medications and treatments as ordered by the physician. Patient and Family Education: Educate patients and their families on disease prevention, management, and overall wellness. Preventative & Chronic Disease Management: Provide guidance on preventative health measures, including immunizations, screenings, and lifestyle interventions. Offer proactive management of chronic conditions, coordinating care with other healthcare professionals when necessary. Record Maintenance: Maintain accurate patient records in electronic medical records (EMRs) and ensure compliance with HIPAA regulations. Team Collaboration: Collaborate with other healthcare professionals, including physicians and nurse practitioners, to ensure continuity of care and optimal treatment outcomes. Quality Improvement: Participate in quality improvement initiatives and stay up-to-date on best practices and clinical guidelines in aesthetics and nutrition. Qualifications Primary care and health assessment skills Experience in chronic condition management and preventative care Ability to develop care plans and coordinate with a team of healthcare professionals Strong communication and interpersonal skills Ability to work independently and provide on-site care Experience in geriatrics or long-term care is a plus Master's degree in Nursing and active Nurse Practitioner license in Ontario Requirements 2 years of clinical nursing experience, preferably with a focus on primary care, urgent care, or geriatrics. Master of Nursing or equivalent, with a clinical focus. Completion of a Nurse Practitioner program (MN or post-master certificate). Registration with the College of Nurses of Ontario in the RN (Extended Class). Strong communication and interpersonal skills, particularly with older adults, families, and multidisciplinary teams. Experience or strong interest in geriatric care and preventative medicine is preferred. Strong clinical assessment and consultation skills. Excellent communication and interpersonal skills for patient education and team collaboration.
