Job Summary: Provides a full range of mental health outpatient services including assessments, treatment planning, psychotherapy (individual, family, and group), crisis intervention, case management, utilization review, and discharge planning. Job Duties: Provides culturally sensitive psychotherapy supporting patient treatment plans and progress monitoring. Performs psychosocial assessments and collaborates with patients on treatment plans. Addresses suicide and high-risk behaviors per standards. Provides education to patients and families based on needs. Performs case management and utilization review as required. Develops timely discharge plans and facilitates referrals based on community supports. Documents services adhering to all professional and regulatory standards. Minimum Qualifications: Master’s Degree in Social Work, Counseling, or related field. Skill in clinical assessment and therapeutic techniques. Knowledge of behavioral health resources. LCSW or LPC license in Pennsylvania upon hire. Physical Demands: Ability to lift and carry 25 lbs., frequent sitting/standing, keyboard use, and role-specific activities such as kneeling, bending, squatting, and CPR.
