The Post-acute and Continuing Care (PACC) team at Jurong Community Hospital (JCH) comprises physicians with postgraduate training in family medicine, geriatric medicine, or internal medicine, providing inpatient care to patients requiring sub-acute or rehabilitative care after an acute illness or surgery. The incumbent will work with a multidisciplinary team of nurses and allied health professionals to deliver holistic care to JCH patients. They will also collaborate closely with community health service providers to facilitate care reintegration into the community. Requirements: Candidate must possess a basic Medical Degree and postgraduate qualifications registrable with the Singapore Medical Council. Applicants with MMed (FM), FCFPS, MMed (Int Med), or other postgraduate qualifications recognized by the College of Family Physicians Singapore (CFPS) or the Specialist Accreditation Board (SAB) will be considered for Senior Physician or Specialist positions.
