Job Description
Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Hospice Duration: 12 months Shift: 8am to 5pm, potential for on-call 5pm to 8am 1-2 times per month Hours per Shift: 8 hours Experience: Minimum of one (1) year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered as experience in individual cases. License: Current license to practice nursing in the state of employment Certifications: BLS (American Heart Association), current automobile insurance, and valid driver’s license Must-Have: Knowledge of terminally ill patients and their families, understanding of hospice services, excellent patient assessment skills, thorough knowledge of managed care principles, regulatory guidelines, and effective operation of nursing assessment tools and technology Description: The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family. The RN will manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care. The RN will case manage all assigned patient clinical records. The RN assures the highest quality of care is provided and documented in accordance with the Plan of Care. The RN performs initial and ongoing assessments of patients. The RN helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with other members of the Interdisciplinary Group. The RN works closely with other members of the Interdisciplinary Group to provide all-inclusive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support to patients and families. The RN reports any changes in the patient's condition to both the Attending Physician and Hospice Physician. The RN maintains accurate and updated medical records, including all Interdisciplinary notes, Plans of Care, initial and ongoing assessments, and any other information pertinent to patient care. The RN practices infection control measures and standard precautions. The RN monitors the scheduling of all staff. The RN participates in the Quality Assessment and Performance Improvement Program. The RN may be requested to participate in an agency on-call schedule. The RN consistently promotes the company's core values. This is a safety-sensitive position. The RN completes required compliance annual training and performs other activities as assigned. The RN attends staff meetings and maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process. The RN makes routine supervisory visits of, and with, all other team members at least monthly. The RN performs ongoing review and follow-up on assigned patient clinical records to assure quality services are provided, standards of practice are followed, and compliance with policies, procedures, and regulatory requirements is met and maintained. The RN maintains close contact with the patient’s family and/or significant others to provide information, support, and continuity of care. The RN maintains primary control and professional management of each patient and acts as the primary liaison between physician, patient/family, and the hospice team to be fully aware and abreast of all clinical issues, staffing, and scheduling concerns. The RN is responsible for the education of the patient, family, or other caregivers in relation to needs identified in the Plan of Care, including medication administration, dressing changes, oxygen administration, hospice philosophy, and care of the terminally ill patient.
