TDAP, MMR, Varicella, Hep B, COVID-19, and Flu (Oct?Feb) ? CMEs/CEUs (past 2 years) ? 9 hours each in Stroke & Trauma annually o Providers at STHS Edinburg/Children?s must complete 27 trauma CMEs over 3 years ? Education/Training Diplomas ? must have completed residency in EM, PEM, FM, IM, Surgery, or Critical Care ? Social Security Card ? TX Driver's License ? Passport-sized Color Photo ?
