Nurse Practitioner - Primary Care Join a dedicated healthcare team as a Nurse Practitioner at a primary care outpatient clinic in Philadelphia, PA. Job Details Employment Type: Permanent Schedule: Monday - Friday, 8 AM - 5 PM Location: Philadelphia, PA Job Requirements Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC Active, non-probationary state Nurse Practitioner license Active DEA license US Work Authorization At least 1 year of experience in primary care, internal medicine, geriatrics, or family medicine preferred Responsibilities Provide exceptional primary care and conduct office visits for routine and acute issues Administer annual wellness visits and health risk assessments Coordinate care with other providers, specialists, and testing facilities Lead population health initiatives in coordination with the Care Team Assist with phone triage and outreach Educate patients on their health conditions, care plans, and treatments Participate in promotional activities and conduct home visits as needed Apply now to join our Philadelphia healthcare team and make a difference in the community!
