This role offers an incredible opportunity for a compassionate Nurse Practitioner or Physician Assistant (NP/PA) to make a meaningful difference through intake assessments at an intake center in the Bronx. Position Highlights: Schedule: Monday-Friday Hours: 4 PM-12 AM Setting: Intake Center Position Type: Full-time, permanent Key Responsibilities: Conducts comprehensive medical intake assessments for new clients Gathers patient histories and performs focused physical exams Identifies urgent clinical needs and coordinates appropriate care or referrals Accurately documents findings and treatment plans in real time Collaborates with interdisciplinary teams to ensure quality and continuity of care Qualifications: Active NYS PA or FNP license Experienced Providers or New graduates welcome-supportive onboarding provided Strong communication skills and a passion for working with underserved populations This role offers an incredible opportunity to make a real difference while growing your career in a supportive and mission-driven environment.
